Need a special Voting Plugin

Hi,
Special need here. I do an illustration for each post. I need a way to place a multiple choice question in a post so that visitors can choose an answer to the question about the illustration.
Such as:
…ILLUSTRATION HERE…
This man is:
(radio button)Angry
(radio button)Happy
(radio button)Sad
VOTE
….RESULTS HERE….

Results need to be compiled and stored in the database.

If the person has already voted, the results are visible and they are restricted from voting again.

When someone is viewing the post for the first time the results are hidden. Upon submitting their answer, the stats are revealed showing the answers in order of popularity with a count or percent of bar chart etc.

The blog will be full of these posts with a different poll for each post.

Any ideas???
