need a form-plugin

  • Hi all,

    I’ve bin searching a lot for a specific form-plugin.

    I want people to order a product (meat). So first, there has to be a textbox where people can put in the quantity or weight. Behind the textbox must be a discription of the product.

    When filled in it must be send by mail to me and the customer. But, in that form there may only be the filled in fields and not the entire form, so empty field must be invisible.

    Does anyone know a plugin which covers that?
