Need A Contact Form to use in a theme or widget ! ?

    I am looking for a contact form that I can put on my front page or in a widget. Any advice ?

    Most of the contact forms that I found you could only implement in a post or page!

    An example contact form from a different page (close the add :):
    autolekar.pl – on the right they use a form and I would like to have something similar.
