Name of students on worksheets

Hi,

Is it possible to have the names of students on a cover page when someone downloads our worksheets?

The front page of the PDF will have this text:

This booklet belongs to

We want parents to enter their child’s name when downloading the file and have the name appear on the front page like this:

This booklet belongs to (NAME)

Please let me know.
