name, nationality raname

Hi, i really like your plugin, thanks! is there any posbility that you can help me out on a situation, please?

i just realise that for some time pictures are not renamed anymore, all picures was renamed automatically in media after post name and nationality from categories,

example;
this was perfect

original pic name = 81A7171web
renamed = novah-e-european-escort100112.jpg

now i did try based on the title media enabled On Upload but not luck

based on atachements post
ACF Field Name = could please tell if here need to add something so rename can modify extension like before (original pic name = 81A7171web
renamed = novah-e-european-escort100112.jpg)

By default, your files will be named after the title of your media entry.
Not doing that anymoare ..
