Name, email doesn't show up

Jetpack’s Comment form is indeed designed to work this way: by default the comment fields are hidden, and will only appear once you click on the comment field.

If you’d rather use a comment form that’s completely visible by default, I’m afraid I can only suggest that you go to Jetpack > Settings in your dashboard, and deactivate the Comments module. Unfortunately there is no easy to change Jetpack’s Comment form behaviour at the moment.
