My apply now form doesn’t work only if I attach a file

My apply now form does not work if I try to attach a file(one of the field in the form), but works fine if I don’t attach the file. I have checked the form tags [file your-filename] it is fine, can’t figure out where the problem is. It was working file but all of sudden, problem occurred.
