Multiple Uploads Won't Work

We have a form here:
https://www.aianwpr.org/2013-rda-upload/There are three file uploads. When I hit submit, a modal window appears saying “Uploading” then nothing.
This is a huge issue as the plugin used to work. We have not made any changes to either WordPress Core or your plugin.
