Multiple Text Input Fields

I’m building a form with these specs:
3 Normal contact fields (Name, email, Contact info)
5 questions.
5 Text box input fields (One after each question)

I would like to have one submit button that would send all of the fields on one email. What I have is one button that only sends the last text box field.

Is this possible?
