Multiple Sub Labels and fields under a main label

Hi, I would like to create an address section on the form with the label “Venue” then followed by multiple fields with the labels “address 1”, “address 2”, “Postal Code”, etc. Right now I can only do either a “text” or “textarea” field type for the “Venue” label, and responders are not giving me their full addresses. How can I go about doing this? Any help is appreciated. Thanks!!!
