Multiple Line Items – Importing Manual Orders with CSV File

Loving your plugin so far.

I have three questions, in order of importance:

1) I need to add manual orders so that I can send manual reviews to past customers before I installed the plugin. Many of my orders have multiple line items that they purchased. How would I format the CSV file to include multiple line items?

2) When I send out a coupon after a review has been submitted, the coupon will be automatically linked to a page such as: https://domainname.com/discount/6hky-ykfh-k1e3?utm_campaign=judgeme-review-coupon&utm_medium=email&utm_source=judgeme

This page does not exist on my site. However, I have double checked and the coupon does work when used in the cart. Is there a way to remove this link or change it?

3) I would prefer to not have a Review Title at all and the customer to only have the option of filling out the comments section. Anyway to remove this field?
