Multiple entries of the same field

    Whenever I make a custom form with City, state, and zip as multiple different fields and each time I download the submissions the information for those fields are generated in random areas of the CSV file and there are multiple City, state and zip fields when in the form there is only one of each field. What could be causing this problem?
