Multiple check boxes (different list) in custom form

Hi,

So I have to different lists in my mailchimp and I would like to use the checkbox from each list in two different forms. Lets say the lists are L1 and L2 and the forms are F1 and F2. I want to connect L1 to F1 and L2 to F2. I went through the documentation and it states to include the following:
<input type=”checkbox” name=”mc4wp-subscribe” value=”1″ />
My question is how can I specify the lists name or id in the above tag?
