Multiple ads displaying per post

We’re trying to set up several OpenX ad zones to display mid-article on some sites. We have six different zones for six different size formats. I’ve configured the six zones, but if there are multiple campaigns running, instead of serving up just one ad, two ads are being displayed currently.
Also noticed that the plug-in is generating blank fields on the posts even when there is no ad to display. I can see this in the code when I inspect it.
