Multipart Comments

Hello,

I would like to add a feedback page for my service that is open for everyone to read. I would like to do this by asking three questions — what did you like? dislike? etc — and then have the answers remain visible on the page, similar to a comment. I don’t think a typical form would work because although I would get the feedback sent to me, it won’t automatically be available for everyone else to see.

Does anyone have an idea on how I can do this?
