Multibyte filenames – ack!

If I update files to the Media Library that have multibyte characters (e.g. Japanese or Chinese), those characters are garbled by WordPress or my server. I am in Japan, so even if I tell my site editors to never upload anything with Japanese in the filename, I know it will happen.
I tried the WP Multibyte Patch plugin, as it claims to replace multibyte filenames with their MD5 hash (tacky looking, but better than what is happening now). But on my site it doesn’t appear to do anything.
Does anyone know of a solution?
