Mult-page form not separating field groups across pages

I’m working with the multi-page form and even when cutting and pasting the short code from here (https://xnau.com/creating-multiple-page-forms-in-participants-database/) I get odd results. The first page of the form acts correctly (showing only the “main” Field Group), but then the second page shows both “address” and “personal.” The third page then shows only “personal.”
When I tried setting up my own mult-page form with my own field groups, they all show up on the second page, even though I’ve specified only one.
What am I doing wrong? Thanks.
