moving contact form

By customer contact form are you referring to vendor registration form?
In that case, the whole form comes with it’s own shortcode i.e. [vendor_registration], that display both the woocommerce fields and the custom from field too.

Hi @marty2010, thanks for the further clarification.

However, this widget doesn’t have any shortcodes. But you can create a shortcode and add the functionality there.
Or you can simply refer to our code here and add that to your site by adding further custom code: https://github.com/wcmarketplace/dc-woocommerce-multi-vendor/blob/master/templates/widget/quick-info.php
