Move "Choose a contact", select width and remove "Acceptable file types…"

    In regards to your third question go to Fields tab and towards the bottom you will see the following option Attached files acceptable types:

    Hi, so what you want to do is more one field from the top to the bottom is that correct? If it is have you tried to drag the field from the top to the position you want and then clicked on Save Changes button?
