Modal dialog not displayed every time my page is loaded

Two things to change:

– Check the box that says: Only show on specific pages and single posts
– For the two fields specifying where to display the dialog, you need to specify page ID, separated by commas (e.g. 3,4,6).

You can get page IDs easily by installing a plugin like Reveal IDs, then go to Page list and get the IDs.

Please confirm that this works correctly and consider donating to support this plugin’s development.

That looks better. So, 282 is the ID of your contact page correct? Can you check in your theme files that you have a call to the wp_footer function in your footer.php file?
