    Is your blog page the front page of the site? If so, make sure you check the box to display on front page in the configuration window.

    Can you perhaps send me a screenshot of your Modal Dialog configuration so I can see how you have set it up so far? You can use a service like imgur.com to upload the image and get a share link easily.

    Two things to change:

    – Check the box that says: Only show on specific pages and single posts
    – For the two fields specifying where to display the dialog, you need to specify page ID, separated by commas (e.g. 3,4,6).

    You can get page IDs easily by installing a plugin like Reveal IDs, then go to Page list and get the IDs.

    That looks better. So, 282 is the ID of your contact page correct? Can you check in your theme files that you have a call to the wp_footer function in your footer.php file?

    Your footer code looks fine, and your configuration looks all right now. But you are still missing some output from the plugin in your page footer.
