Missing icons in fields

    Hello!

    Sorry for the delay, this might be a theme issue (your theme is trying to overwrite the font that the icons are trying to use which is needed to correctly display them), could you change your theme temporary and see if the icons are shown that way?

    I have just turn off the icons and problem is solved in half way. How to figure it out where is problem in my template and smartform plugin?
