Missing/broken functionality in backup storage selection

All of the information and functionality under “Choose your remote storage” is reduced to 18 unlabeled checkboxes. I’m in the middle of a ground-up site rebuild, and when I try to mimic the settings on the existing install (the error is present on both sites, and I’ve tried troubleshooting mode with no improvement) to back up to FTP, the new site throws an error that I haven’t configured the destination, which makes sense because the block where I’m supposed to set it doesn’t display when I check the necessary box. (It doesn’t for any of the other methods either.)

I’ve been a user of UD for a long time, and this problem has probably existed for a couple of years but I didn’t care because it wasn’t breaking my work processes. Unfortunately, now it is.

I’ve tried exporting/importing settings as well. No love.

If I can’t fix this I’ll have to find another plugin to do the job, and I’d really prefer not to if I can avoid it. Whatever insight you can provide will be appreciated, and thanks for what has (other than this) been a perfect tool for a long time.
