Mini shout type box

  • Hi all.
    I have a site where I would like users to be able to leave a two line message (name and message). Very similar to the standard comments form. But I need this both this AND the usual comment form.

    At the moment I am thinking of somehow duplicating the comments function, but could do with a firm point in the right direct.

    Any help greatly appreciated
