Messages with no info being sent

I keep getting submitted emails with just this info:

%%Name%%
%%Email%%
%%Message%%

The 3 fields I am collecting are all required and there is a math captcha required. Email is looking for a valid email adress.

I am not sure what is going on. This site is a few years old and I have updated all the plugins.
