Memberships auto approving

Lately, anyone who registers on our site is being auto-approved, and the registration form is creating them an account, even though they get no confirmation on the page. Bascially, anyone can go to our Registraiton page, create an account, and when they hit the Register Button, it refreshes that page, with no redirect or info given AND automatically creates their account (without it going into Pending status)

I have verified that User Roles -> Require Admin Review
I have verified that Anyone Can Register is unchecked on WP Settings
I have verified that the Your Account is Pending Review email is setup.

Running WP 5.1.1, Ultimate Member Version 2.0.43

Any ideas what could be happening? Has worked great for years; quit working within the last 6 months.
