    I just wondering is it possible each member to view his/her own ticket only and they don’t be able to see each others ticket?

    That is the default behavior. Only admins can see all tickets.

    also is it possible to add more fields to the form

    We’re working on adding this to the PRO version in the near future

    send automatic reply to them

    Any time you or your customer makes or replies to a ticket, you both receive an email.
