Member page creation and editing

I want to allow certain members to fill in a form and create a page from a template (with images, etc). The page (for businesses) is to last a calendar month before expiring. I would like the person to have a limited control over the page to allow for editing. Maybe just from reposting the form.
Of course I need control in admin to approve pages and changes. Can you recommend a plugin? It should be bug free/ established and scalable. Thanks.
