Member Groups not displaying

Replace if block at line no 354 in bp-group-hierarchy/extension.php with following if block

if(isset($_REQUEST['grpage'])) {
   $page = (int)$_REQUEST['grpage'];
} else {
   $page = 1;
}

Modifying the if block as Paresh suggests solves this problem.
