Media Uploader refreshes when I click "Insert into Post"

    Hey Friends,

    I have a very frustrating problem when using the media uploader. I upload an image as usual, then when I click “Insert Into Post” instead of it inserting, then Media Uploader refreshes and does nothing.

    I have tried to completely remove and re-install WordPress, I have no plugins activated, and I have the latest version of WP too.

    Will gladly provide admin access if someone can please please help me.
