Media names in pages & posts not replaced with new file name

When replacing an image with one of a new name (using the option: ‘Replace the file, use new file name and update all links’) only the hyperlink on pages and posts is replaced. The old file name remains.
This is what happens where photos are placed on pages and posts – observe the file names:
