Media Library uploads not reliably working

Hello,

We have been experiencing an issue with media uploads in Caldera Forms.

We have File upload fields on a couple of forms that are set to upload into the Media Library in WordPress. We have noticed that images do not properly upload into the Media Library on form submission. An email gets sent with links to the uploaded files, but I’m getting 404 errors when I try to view the uploaded files on the website.

I see the uploaded files show up in the Media Library, but with empty thumbnails. Nothing actually gets uploaded into the /wp-content/ folder.

I did some troubleshooting with our host (WP Engine), but there doesn’t seem to be anything wrong server-side. I have also done some troubleshooting with other plugins disabled, and it does not seem to be a plugin conflict either.

In addition to this, we have noticed a bug when submitting the form via Apple/iOS devices. The form gets hung “submitting,” with a spinning icon, but it never goes through. Not sure if these two are related, but it’s worth mentioning.
