Media Library not showing uploaded media when the plugin is active

When I activate the plugin, I can’t see the uploaded media files in media library. Also When I try to add a media to a post, I can’t see the uploaded media.
When viewing the Media Library, When I choose mode=list, everything is fine, but when I choose mode=grid, the media files don’t load.
