Media files keep uploading to wrong directory, e.g. wp-content/uploads/2014/07/

Hello Flywheel Support,
For some time now, everything I upload to neactioncoalition.org generates a name such as https://neactioncoalition.org/wp-content/uploads/2014/07/Defining-and-Transforming-the-Culture-of-Health-in-Nebraska-2015-1.pdf. This just happened . What could be causing the filename not to reflect the current year and month? I went into SFTP and saw that it is actually stored in both places uploads/2016/04/…..Please advise, thanks.

Hey Ava,
I did some digging and found that it is the Google Forms plugin that is causing the issues with the file uploads not going to the appropriate directories. You might contact the plugin developer to see if they know what is causing this, but the temporary fix is to deactivate it. Let us know if you have any questions.
