Markup and Wrapping Div

Thanks for this great plugin. It’s been very helpful and straightforward.

I’m wondering two things:

1. Would you be willing/able to add some markup around the entire footnote block at the bottom? The OL is classed well and easy to affect with styles, but when you add footnote header or footer text, it is wrapped with nothing.
2. The second thing is it would be helpful to wrap the individual header and footer messages with some html markup.
