Map only showing one tile suddenly

  • I had installed the map in the form, set the initial point, and all was working well.

    Coming back to the page today, there is only one tile showing in all browsers. I have reinstalled all the plugin files, taken out any customization, and tried it in every browser. One tile. When I right-click “inspect”, the full map shows.

    The page is https://estero-fl.gov/public-works-test/ and the password is pwtest.

    Any help would be appreciated.

    I tried re-creating the form and checking the map every time I added a new question. It appears that if another question in the form enables conditional formatting, it breaks the map. Any ideas on how to avoid this?
