    I checked the forum for similar topics but couldn’t find the exact problem I am having.

    This is the example of wordfence report I am receiving lately:

    * File contains suspected malware URL: /home/username/public_html/wp-content/cache/page_enhanced/www.915my.com/_index.html

    When I login with FTP to server and see cache folder, I see a dozen of similar spammy folder-domains besides my domain name (which should be the only one there, right?). All these spammy folder-domains have only _index.html file in it. The owner and the group of these folders is apache.

    When I check source of those html files, it says:

    Served from: www.915my.com @ 2016-02-01 16:30:02 by W3 Total Cache -->

    Now, how can this be possible? To serve pages from domains which are not mine, nor linked with my website by any means?

    I suspect the website is hacked, but still trying to figure out if this is the case, and if it is, then how.

    I don’t know how W3TC exactly works, but any feedback which could lead me in the right direction will be helpful and appreciated.

    Sorry can’t give that.

    But as a follow up, What changed in the meantime is instead of many spammy domain.com folders inside cache folder, now there is only %s, and when I check _index.html it says:

    Served from: %s @ 2016-02-04 07:24:54 by W3 Total Cache -->
