Make “Submit” button follow previous fields

Hello, I am trying to make the ‘submit’ button at https://65medicare.org/turning-65-list-medicare-supplement-plans/ follow the previous fields. I’m sure there is an easy way to get it inline with the previous fields but I cannot figure it out. Any help is appreciated. In other words, I’m hoping to have it all on two rows – three inputs on the top row and two inputs and one submit button on the bottom row. Thanks in advance.
