Main post Image

I have been looking for a plug-in to do this for ages now, and there are some that suggest that they can but I have yet to find one that works.
I would like people who are writing posts to be able to upload an image to be the main image for that post. This image must meet minimum dimensional requirements (and ideally ratio requirements) as well as be under an upper file size limit. It needs to be simple.
I need it to work on WordPress 2.7 can use php5.
I know there are a few plug-ins out there that can scan the post for an image and use the first one to represent the post, but I don’t really want this image to be part of the post itself, and none of them are able to restrict the dimensions or ensure the correct ratio.
I think the best place would be to store the image URL in a custom field, but I want the interface to be really simple. I don’t want to have to ask my users to upload an image to the FTP server, hope its the right dimensions, work out the URL, add the custom field and past in the URL; far too much room for mistakes.
