Mailchimp custom integration html not adding check box

  • Hi using the custom HTML to add a custom integration to a form in Custom Contact Forms. I added the following HTML as per the instructions with the plugin:

    <label>
    <input type=”checkbox” name=”mc4wp-subscribe” value=”1″ />
    Subscribe to our newsletter. </label>
    </p>

    The “Subscribe to our newsletter” text shows up on my form, but no check box shows up. Any idea why it wouldn’t be visible? My site isn’t live so I can’t link to it.
