    A really excellent free plugin, easy to use out of the box and does exactly what I wanted. Highly recommended.

    Hoping to style the look of the form I purchased the Custom CSS add-on (only USD5) which is fine for easily changing the background & text colour of the header & submit button, but other than that lacks CSS class selector details for changing, for instance, the shape/size of elements. I managed, but anyone without CSS experience may struggle a bit.
