"Lost Password" and other error messages don't appear

Hi there

We’re having some issues. The “Lost Password” link doesn’t appear on any sites we’re using your plugin on.

Also, error messages, such as those that would appear after 3 unsuccessful login attempts, simply don’t appear.

Any ideas what’s happening here?
