Looking for plugin – multi select form with results

  • I’m looking for a plugin where you can choose options, for example in radio buttons, and get results based on what you chose. Kind of like a “who are you” quiz where the results are based on your chosen inputs.

    Does anyone have any tips? or know what this is called that I can search?
