Looking for a specific contact form plugin

I am looking for a contact type form that would allow auto response within the form. They will answer a list of questions and then depending on the answer they give for those questions the form will inform them of what they are looking for. The results should come up on the screen after they submit like for example, a mortgage calculator or something like that would give results for each person after filling out the necessary information.

Does anyone know if there is a plugin for that somewhere?
