Looking for a plugin that emails a form to the user who fills it out.

The title of the topic pretty much sums it up.

I’m looking for a plugin, if one exists, that will allow a visitor to fill out a form on the website and when they click submit the form is then emailed to them at the address that they supply in one of the form’s fields.

Does this exist? Any suggestions?
