Lockouts Not Showing in Log

I have reports of both an Admin and a customer user both getting locked out due to too many attempts (4 attempts, 20 min lockout, 4 attempts, 8760 hour lockout)
I’m sure someone or bot has caused this, but neither of these lockouts shows in the plugin log.
Any thoughts?
