Load dynamic content in page

Hi, I would like to know how can I have custom code in PHP render on a page? I have an external database which i would like to have a page pull data from. Is this possible?

In addition, I would like to have the form which populates this data, appear to be integrated within what ever template I have running, is there a way to do this as well?
