List Item on how to set limit

Hello sir,
I have created one option “contact info” with “List Item types”.added “contact first”,”contact second”,”skypeid”,”skype url”(with text options).
Now display on options page well and added with add new button.
why display add new button again or again (there need data one time only). see the screen.
