Links to related images keep disappearing from my posts

I have posts with multiple related images and every so often I lose the links. For example, I would put in my post:
<img title=”Teak Root End Table Cube 2″ src=”/images/teakresintable002t.png” />
And the image teakresintable002b.jpg will be related in an album [teakrootcube2] with teakresintable002a.jpg, until a few weeks later I work on the site again and the second part is missing but there is empty space in the html view of the post.
