Limit per IP for submitting forms?

I would like to add some limits per IP to my submission form. I want to allow let’s say 5 submissions in 1 hour per 1 IP. If a person with the same IP tries to submit for the 6th time within the same hour, he/she would get blocked and allowed to submit again only after 1 hour or something like that.

Maybe there is a good WP plugin for this? Or a custom solution? ??

EDIT: I also would like to get an option which shows me from what IP a visitor submitted a form. I want to track each IP. Any good ideas on this would be appreciate.
