limit category choice?

I can’t seem to get it to work. 2 problems, 1. save form says form is saved but then warning about unsaved page when I try to leave.
2. include doesn’t work, I listed desired categories and all are shown.

Did I do it wrong? To select one category from a limited list?

1. The message just notifies the user when leaving the form page, it shows all the time whether the form is saved or not.

2. I have checked your screenshot, it seems you include the name of the terms instead of the term IDs. Please check the following screenshots:

You should enter the term IDs as comma separated (without space) to exclude/include in the form.
